Two pricing structures: (1) Membership - includes annual complete physical exam, 24/7 phone/portal access, unlimited follow-up visits at $25/visit, home visits at discretion, discounted labs, no charge for prescription refills or prior authorizations, priority scheduling. Minimum initial down payment of 2 quarters required for new patients. (2) Fee-For-Service (FFS) - $100 per 20 minutes for established adults (23+); $70 per 20 minutes for established children/students (<23); New adult patients: $200 for first 40 minutes then $100 per additional 20 min; New child patients: $200 for first 40 minutes then $70 per additional 20 min; Complete physical exam: $300 for adults 23+, $210 for ages 1-22; Phone consult for established patients: $30/call for 1-2 problems. After-hours rates 20% higher.
