Adult membership costs $2,000 per year. This flat annual fee covers unlimited visits, a comprehensive physical, cancer and disease screening, chronic disease management, and health, nutrition, and fitness planning. Child and dependent memberships cost $1,000 per year for children or grandchildren of adult members between ages 3 and 26. There are no copays and no hidden fees. House calls carry an additional $150 fee, and after-hours treatment runs $60 if care is required.
