About

At SAGE Direct Primary Care, Dr. Ella Shadmon delivers full-scope family medicine from her South Pasadena practice. She founded SAGE in 2026 after 23 years in the same insurance-based medical group. Her goal was straightforward: bring back direct, relationship-driven primary care.

The name SAGE carries two meanings. It stands for Supportive environment, Accessible physician, Guided care, and Excellence in every decision. It also references the sage plant, long valued as a symbol of healing. Dr. Shadmon holds board certification in Family Medicine and brings particular depth in midlife medicine and obesity medicine. She manages chronic conditions including hypertension, diabetes, cholesterol, and thyroid disorders. She also prescribes hormone therapy for perimenopausal and menopausal women and supervises medically managed weight loss including GLP-1 medications. Visits run 30 to 60 minutes, and members reach Dr. Shadmon directly by call, text, or email between appointments.

Yes. SAGE membership covers your primary care with Dr. Shadmon. It does not cover specialists, hospital care, imaging, surgeries, emergencies, or most prescription medications. You need insurance for those services, and Dr. Shadmon will help you use it well. The membership pairs particularly well with a high-deductible health plan.

Medicare patients are welcome at SAGE. The practice has opted out of Medicare billing, so Dr. Shadmon does not bill Medicare for services covered by the membership. Members sign a waiver confirming that neither they nor SAGE will submit claims to Medicare for care received here. You keep your Medicare coverage for prescriptions, hospital care, labs, imaging, and emergencies.

Yes, SAGE accepts members with HMO plans. One important note: HMO plans require an in-network primary care provider to issue referrals for specialists and other covered services. SAGE cannot serve as your official in-network PCP for that purpose. Many HMO members keep their in-network PCP for referrals and come to SAGE for their primary care.

For routine visits, SAGE typically schedules within days. For urgent needs, Dr. Shadmon offers same-day or next-day appointments. This means you can see Dr. Shadmon for a UTI, an acute illness, or a rash that needs attention today, rather than waiting weeks or visiting an urgent care clinic.

Appointments at SAGE run 30 to 60 minutes. SAGE does not schedule 15-minute visits. The longer format allows Dr. Shadmon to cover everything you bring to the appointment and hold a full conversation about your health without cutting it short.

Members contact Dr. Shadmon directly by call, text, or email. There is no call center and no message queue. She responds to questions, new symptoms, prescription refills, and referral requests in a timely manner. Telehealth visits are also available through the SigmaMD patient portal.

Membership covers in-office and telehealth visits, same-day or next-day urgent care, preventive care and annual visits, chronic condition management, perimenopause and menopause care, weight management and metabolic health, GLP-1 medication management, acute care, prescription refills, referral coordination, and some in-office testing. Direct access to Dr. Shadmon between visits is also included.

Yes. SAGE offers a free 15-minute meet-and-greet with Dr. Shadmon before you join. It is a no-obligation conversation. Contact the practice to schedule one.

SAGE requires 30 days notice to cancel a membership. The practice charges no cancellation fee.
