Perennial offers two membership tiers. Advanced Longevity costs $500 per month, billed monthly, and includes four scheduled physician visits per year, plus DEXA, VO2 max, and resting metabolic rate testing. Perennial Concierge costs $2,100 per month and includes unlimited physician visits, unrestricted direct access, urgent visits, the same core diagnostics, and full specialist navigation. New patients start with a $695 Longevity Baseline Assessment, which credits toward membership if ongoing care is chosen. Outside labs and imaging bill separately. HSA and FSA funds may apply.

New patients begin with the $695 Longevity Baseline Assessment. This is an unhurried physician visit that reviews your history, goals, prior labs, medications, family risk, sleep, training, and nutrition. Dr. Mottram uses that session to build a written baseline plan and recommend whether ongoing membership fits. Some patients continue into membership; others leave with a plan and return later.

Visits at Perennial run 90 minutes. Dr. Mottram uses that time to interpret diagnostics, wearable data, and outside records within the full context of each patient's history and goals. The practice keeps its panel at 200 members so that visit length stays meaningful.

Perennial operates as a direct-pay practice and does not bill insurance for membership fees. Outside labs and imaging may bill separately through insurance depending on your coverage. HSA and FSA funds may apply to certain costs, depending on your plan.

Yes. Dr. Mottram offers telemedicine visits for members. He cares for patients across Wisconsin, multiple U.S. states, and the United Kingdom, so remote access is a core part of how ongoing care works.

Perennial uses advanced biomarker panels including ApoB, Lp(a), fasting insulin, and inflammatory and metabolic markers. The practice also offers DEXA body composition scanning, VO2 max testing, resting metabolic rate testing, continuous blood pressure monitoring, home sleep studies, whole-body MRI, multi-cancer early detection with Galleri, colorectal screening with Cologuard, and genetic and pharmacogenomic testing. Dr. Mottram selects each diagnostic because the result can change the clinical plan.

Perennial caps its panel at 200 members. Dr. Mottram set that limit so access remains real and every patient's full story stays known to their physician.

Perennial OS is a proprietary patient platform that Dr. Mottram built himself. It keeps lab trends, care priorities, messages, and rolling care plans connected between visits. Members use it to stay engaged with their plan outside of scheduled appointments.

Perennial integrates data from several wearable and monitoring partners, including Hilo for continuous blood pressure monitoring, Oura, continuous glucose monitors, Withings, Steloby, and Dexcom. Dr. Mottram reviews this continuous data as clinical context alongside labs, diagnostics, and each patient's history.

Yes. Dr. Mottram cares for patients across Wisconsin, multiple U.S. states, and the United Kingdom. Telemedicine makes ongoing care accessible for members who live or travel outside the Middleton area.
