Our Family Direct Primary Care

Monthly membership fees start at $30 per month for children enrolled with an adult and $65 per month for adults ages 20-44. Adults ages 45-64 pay $90 per month, adults 65 and older pay $110, and families of up to two adults and four children pay a maximum of $195 per month. Employees enrolled through a participating employer pay $60 per month. An enrollment fee applies at signup.

Each membership covers up to five office or technology visits per month, same-day and next-day scheduling, an annual physical exam, and standard in-office procedures. Members also get expanded access to Dr. Manire by phone, text, and email, including after-hours access via his personal cell phone. Wholesale labs, imaging, and medications are available to members at additional cost.

Yes. Our Family Direct Primary Care accepts payment from Health Savings Accounts and Flexible Spending Accounts. Employers can also offer DPC membership as an employee benefit, and the practice partners with employer groups including Phia and Riedel-Hogan CPA for employee enrollment.

Our Family Direct Primary Care does not bill insurance for membership fees or primary care services. The practice operates outside insurance networks, which allows Dr. Manire to keep costs transparent and eliminate copays. Members with existing insurance plans can maintain that coverage for specialist visits, hospitalizations, or other needs beyond primary care.

Members receive same-day and next-day scheduling after completing onboarding. The practice prioritizes members for appointment access. Non-members may also schedule visits, though they receive second-priority scheduling.

Yes. Dr. Manire conducts technology visits by phone, text, email, and video including via Skype. These count toward the five monthly visits included in membership. Members can also reach Dr. Manire after hours through his personal cell phone.

Yes. Our Family Direct Primary Care offers house calls as part of its patient access options.

The practice performs a broad range of in-office services. Diagnostic capabilities include EKG, spirometry, exhaled nitric oxide testing, continuous glucose monitoring, ambulatory blood pressure monitoring, and allergy skin testing. Procedural services include skin biopsies, laceration repair, cryotherapy, ingrown toenail removal, ear wax removal, and incision and drainage. The practice also carries an in-office pharmacy with wholesale generic medications.

Yes. Our Family Direct Primary Care serves patients in Louisville and in Southern Indiana communities. The practice website is available in English, Spanish, and Vietnamese to accommodate a diverse patient population.

Yes. Our Family Direct Primary Care requires patients to stay current on all CDC-recommended vaccinations. The only exception the practice recognizes is a documented medical contraindication.
