Mitten Pediatrics & Lactation uses age-based monthly pricing per child. Children under age two are $195 per month, ages two through five are $150 per month, and ages six through eighteen are $100 per month. Sibling discounts apply automatically: the second child receives 10% off both children's rates, the third child triggers 20% off all, and a fourth child triggers 30% off all. No copays or deductibles apply to primary care services covered under membership.

Membership covers all well-child visits, unlimited same-day sick care, school and sports physicals, chronic condition management, developmental and behavioral screenings, vaccinations, care coordination, and direct phone, text, and video access to Dr. Sebastian. Basic in-office testing is included. Labs sent outside the practice pass through at near-cost pricing.

Families start with a Meet and Greet with Dr. Sebastian before enrolling. You can schedule that visit through the contact page on the practice website. Prenatal enrollment is available for expecting families who want to secure a newborn spot before their baby arrives. The practice maintains a waitlist when its panel reaches capacity.

Mitten Pediatrics & Lactation does not bill insurance for membership or primary care services. The practice operates on a direct pay model with no insurance involvement in primary care. Families keep their insurance for hospitalizations, specialist visits, imaging, and emergencies. Memberships qualify as HSA-compatible expenses under the OBBBA effective January 1, 2026.

The Warm Mitten Package covers physician-led newborn care and IBCLC-certified feeding support in the home during the first four months of a baby's life. It includes four in-home visits combining newborn exams, feeding assessment, and postpartum guidance. The package also includes full DPC membership access during months one through four and waives the enrollment fee at transition to standard membership. Prenatal sign-up is required, and home visits cover a ten-mile radius from the Cascade Township office.

Yes. Dr. Sebastian offers lactation consultations as standalone services with no membership required. In-home lactation bundles and in-office visits are available separately. Any family seeking breastfeeding support, whether a current member or not, can book directly with Dr. Sebastian.

Yes to both. Dr. Sebastian conducts telehealth visits for situations that do not require an in-person exam. She makes home visits for families enrolled in the Warm Mitten Package during months one through four. Direct text and video access is available seven days a week for all members.
