Kozmic Care

Individual membership at Kozmic Care costs $95 per month. A member plus one spouse or child runs $180 per month, and a family of up to five is $270 per month. Specialty programs carry separate pricing: HRT and menopause support starts at $450 for the first month and $100 monthly after that. Weight loss management is $400 for the first month and $100 monthly thereafter. Medication Assisted Treatment for addiction is $400 the first month, $200 the second, and $100 monthly after that.

The membership covers unlimited office visits with no copays. This includes annual wellness exams, well child exams, sports physicals, school physicals, work physicals, acute care, chronic disease management, and a range of in-office procedures. Members also receive four Osteopathic Manipulation visits per year. Labs, imaging, and wholesale medications are available at cost. Pathology fees and certain other charges can be billed to insurance.

Yes, Kozmic Care encourages all members to maintain at least a catastrophic health insurance plan. The practice handles 80 to 90 percent of primary care needs, but insurance covers hospitalizations, specialist visits, imaging, brand-name medications, and other outside services. Members with a DPC membership may qualify for a higher-deductible plan, which can lower monthly premiums.

Yes. Kozmic Care accepts both HSA and FSA cards. You can also pay by bank account or personal credit card and arrange for your HSA or FSA to reimburse you directly. All members must enroll in auto-pay. A 2% fee applies to credit card payments.

Dr. Kozlowski uses RubiconMD to send your case to any of more than 200 board-certified specialists, including cardiologists, dermatologists, psychiatrists, and orthopedic specialists, at no cost to you. If an in-person referral is needed, she can refer PPO members to any in-network provider. Uninsured patients can access a list of specialists offering cash pricing.

Yes. Dr. Kozlowski offers telemedicine visits at no additional charge to members. Many conditions are diagnosable and treatable by phone or video. She can also send prescriptions to the nearest pharmacy when you are away from home.

No. Kozmic Care requires no long-term commitment. Members can cancel at any time with 30 days of written notice. There is no cancellation fee. If you cancel and later decide to rejoin, a $450 re-enrollment fee applies.

If a member visits the emergency department, Dr. Kozlowski contacts the ER physician directly to discuss symptoms and coordinate care. If a member is admitted, she contacts the hospitalist to help ensure a smooth transition when the patient leaves the hospital. The practice focuses on preventive care and same-day access to help members avoid unnecessary ER visits.

Yes. Kozmic Care offers family memberships for up to five people at $270 per month. Families larger than five pay an additional $10 per person.
