At Family & Lifestyle Medicine, two physicians deliver whole person primary care to the Greater Lansing community. Dr. Kimberly Azelton founded the clinic on January 11, 2022, launching with mobile primary care and wholistic lifestyle medicine. Dr. Jonathan Sharley joined her after relocating from the Pacific Northwest. Both physicians hold dual board certification in family medicine and lifestyle medicine.

The clinic operates as a 501(c)(3) non-profit in cooperation with local Seventh-day Adventist churches. Its mission centers on affordable, comprehensive, personalized, and accessible care for all ages. Membership costs $80 per month regardless of age, with no co-pays and wholesale pricing on labs and medications. The multidisciplinary team includes culinary medicine coaches, fitness instructors, health coaches, a dietitian, and volunteer chaplains. The practice serves a 20-mile radius around Greater Lansing and offers same-day visits in clinic, by secure video, or at home. The clinic also runs free community programs including plant-based cooking nights, fitness classes, and health screenings.
