Membership at CLD Concierge Medicine costs $225 per month, or $2,700 per year. The monthly fee covers unlimited visits, unlimited telemedicine, 24/7 direct access to Dr. Lantz-DeGeorge, house calls, hospital and nursing home visits, and all visits for services including weight loss, hormone replacement therapy, mental health, and health coaching. The practice charges no copayments and does no additional insurance billing.

Members receive unlimited in-office and in-home visits, unlimited telemedicine, 24/7 direct phone, text, and email access to Dr. Lantz-DeGeorge, house calls, in-home EKGs, hospital and nursing home visits, health coaching, and specialist visit coordination. In-home lab draws are also available at an additional cost per draw.

The practice recommends that members keep their insurance for hospitalizations, specialist visits, and labs or imaging. Yes. Because CLD Concierge Medicine has no affiliation with major health systems, Dr. Lantz-DeGeorge coordinates care across all systems in the region. She recommends specialists based on clinical fit, not network restrictions. She can also accompany patients to important specialist appointments.

Yes. Dr. Lantz-DeGeorge has extensive experience with older adults and geriatric conditions. She makes visits to nursing homes and rehabilitation facilities and provides at minimum weekly courtesy visits for patients who are hospitalized. House calls also become a standard part of care as patients age.
