Membership fees vary by age. Infants from birth to 24 months pay $200 per month. Children ages 2 to 4 pay $170 per month. Children 5 and older pay $140 per month. Families with multiple children receive a 20 percent discount on additional children, with a family maximum of $365 per month for up to three children. A one-time enrollment fee also applies.

The monthly fee covers unlimited office visits, after-hours support, in-office testing, virtual visits, care forms, lactation consulting, medication dispensing, and care coordination. There are no co-pays, no per-visit fees, and no surprise bills. Home visits for newborns during the first three months of life are also included at no extra charge.
