Balance Integrated Health offers two membership plans. The Balance Plan starts at $125 per month for the first adult member. 50 per month. Children join at $65 per month each. The Harmony Plan starts at $215 per month for the first adult. A one-time enrollment fee of $99 per member or $198 per family applies to both plans. Non-members can schedule an Annual Wellness and Lifestyle Consultation for $349, which includes a 60-minute visit, standard labs, and a 30-minute follow-up within 90 days.

Members receive unlimited primary care visits, including same-day and next-day appointments. Both plans include standard annual labs: CBC, CMP, Lipid Panel, TSH, and HbA1c. The Harmony Plan also adds CRP. Members access Osteopathic Manipulative Treatment at no extra cost, in-office procedures including sutures, discounted supplements, and discounts on medications at cost plus a standard markup. Direct access to Dr. Marzolf via calls, texts, and video is also included.

Yes. Balance Integrated Health provides members with a secure telemedicine app for after-hours urgent concerns. Members can send texts, video chat, and share photos directly with Dr. Marzolf. The practice also offers same-day and next-day in-person appointments for issues that require an office visit.

Balance Integrated Health operates on a direct primary care membership model and does not bill insurance for primary care services. Many members pair their membership with a high-deductible health plan or health-sharing plan for specialist care and hospitalizations. The practice provides discounted cash pricing for labs and medications to help reduce out-of-pocket costs.

Yes. Balance Integrated Health offers an Annual Wellness and Lifestyle Consultation for non-members at $349. This includes a 60-minute visit, standard labs, and a 30-minute follow-up within 90 days. Additional visits for non-members cost $175 per 30-minute appointment. For ongoing primary care, a membership plan provides the most comprehensive access.

Balance Integrated Health caps its panel at approximately 300 patients. That limit allows Dr. Marzolf to spend meaningful time with each person and maintain same-day and next-day availability.

Yes. The practice offers virtual visits through a secure telemedicine app. Members use the platform for quick concerns, follow-ups, and after-hours urgent needs. When an in-person visit is necessary, same-day or next-day appointments are available.

Balance Integrated Health offers health coaching through Trina O'Brien as an optional add-on or standalone service. The practice runs the Reset to Rise 3-Part Workshop Series, held at The Grace Center in Montrose, which covers energy regulation, metabolism, and sleep. The practice also offers the Core Restore 14-Day Gut Reset program. Starting in January 2026, the practice introduces Red Light Therapy, which supports inflammation reduction, wound healing, collagen stimulation, and energy.

Yes. Dr. Marzolf accepts Medicare patients. The practice does not enroll Medicaid patients, as Colorado regulations prevent DPC practices from doing so.
