Membership plans start at $69 per month for the Basic plan. The Standard plan runs $99 per month and includes unlimited visits with MD, DO, PA, or NP providers plus a free annual visit with labs. The Premium plan is $119 per month and adds one free chiropractic visit per month and further discounts. All plans require a $75 sign-up fee. Family and short-term plans are also available.

Peak Wellness Center is the only medical office in Montrose with both in-house X-ray and ultrasound capabilities. The practice also offers chiropractic care, mental health services, and ultrasound-guided joint injections on site. Nine healthcare professionals serve members across a broad range of primary care needs.

Yes. Peak Wellness Center runs a dedicated employer program. Employers pay no sign-up fees, work with a dedicated account representative, and receive simple monthly billing. They can add or remove employees on a month-to-month basis.

Prospective members can select a plan on the Peak Wellness Center website and complete a short sign-up form. A staff member will call to schedule your first visit. New patients complete a patient packet at the office before their appointment. Members manage records, labs, and messaging through the Healow app using practice code FBFADA.

Yes. The practice offers pediatric care and pre-natal care as part of its services. Families can enroll under family plans that cover up to two adults and two children.
