Individual adult memberships start at $99 per month. Couples pay $195 per month, a single child pays $30 per month with an adult membership, and a family of two adults and two children pays $230 per month. There is a one-time enrollment fee of $99 per family. Some services such as hormone pellets, IUD insertion, and Botox carry additional fees.

The monthly membership covers primary care services and 45-minute appointments. It also includes annual labs such as CBC, CMP, TSH, lipid panel, Pap smear for women, and PSA for men over 50. Members receive discounted pricing on additional lab tests, radiology, prescription medications, and supplements, along with access to telemedicine visits and home visits.

Mindy Miller MD PLLC does not bill insurance for membership or office services. The practice does submit insurance information for lab and radiology testing if a patient prefers that route. Many patients choose to pay cash given the discounted pricing available through the practice's contract with Family Health West in Fruita.

Medicare patients are welcome and receive the same care. Dr. Miller requires the monthly membership fee, which Medicare does not cover, and asks Medicare beneficiaries to sign a waiver acknowledging they will not submit the membership invoice to Medicare. Medicaid patients generally cannot enroll unless an employer pays for their membership.

Mindy Miller MD PLLC schedules extended visits of 30 to 60 minutes. Standard appointments run 45 minutes. That time allows Dr. Miller to address concerns thoroughly without rushing.
