Membership starts at $125 per month for patients ages 0 to 26 and $200 per month for patients 27 and older. Family plans range from $350 per month for couples to $600 per month for a family maximum. No. Vitality Family Medicine does not require insurance coverage. You pay a monthly membership fee directly to the practice. The practice accepts patients regardless of insurance status, including those with MassHealth, Medicare, and Commonwealth Connector plans. Your existing insurance may still cover labs, imaging, specialist referrals, and hospital care outside the practice.

Dr. Moak-Blest offers same-day or next-business-day appointments for acute and urgent concerns. She also provides after-hours phone availability on evenings and weekends for urgent matters. Because her panel stays at 300 patients, she keeps time available for timely access.

Yes. Members contact Dr. Moak-Blest directly by phone, text, or email. She provides personal access to all patients, so you never navigate a front desk or call center when you need to reach her.

Yes to both. Dr. Moak-Blest conducts video visits through the Spruce telehealth platform. She also makes home visits for postpartum and new mothers, patients recovering from surgery, and geriatric patients who need care at home.

Dr. Moak-Blest performs a wide range of in-office procedures, including simple laceration repair, skin biopsies, cortisone joint injections, wart freezing, IUD placement, endometrial biopsy, wound care, abscess incision and drainage, and phlebotomy. She also provides Osteopathic Manipulative Therapy and CranioSacral Therapy for newborns with feeding issues. Lab draws through the practice cost roughly one-fifth to one-tenth of hospital or insurance lab pricing, through a partnership with Quest Diagnostics.

You will always see Dr. Moak-Blest. Vitality Family Medicine is a solo practice, and Dr. Moak-Blest does not use mid-level practitioners. Every visit, every call, and every message goes directly to her.

Yes. The practice serves patients from newborns through elderly adults. Dr. Moak-Blest provides newborn care, well-child checkups, school and sports physicals, vaccinations, women's health, and geriatric and end-of-life care. The office includes toys, developmental play items, breastfeeding chairs, and diaper changing facilities, so families can bring children to any appointment.

Vitality Family Medicine maintains a Meditation Suite on the premises. Through a partnership with Meghan Leighton, founder of Awake Minds Yoga of Newburyport, members access private breathwork, yoga, and meditation sessions at a 30% discount, priced at $55 per hour. The practice also hosts exclusive health workshops for members. Dr. Moak-Blest offers a patient treatment video library covering breathwork techniques, musculoskeletal stretches, osteopathic lymphatic drainage, posture correction, and infection management.

New patients begin with a free 15-minute Meet and Greet appointment, available in person or virtually. Dr. Moak-Blest uses this time to answer questions about the practice model, her philosophy, and how membership works. The practice currently operates on a waitlist for Meet and Greets, with scheduling available two to three months in advance. Dr. Moak-Blest is currently accepting newborns immediately.
