Stephens Family Medicine

Stephens Family Medicine offers individual memberships starting at $40 per month for patients ages 0 to 25. Adults ages 26 to 44 pay $85 per month, ages 45 to 64 pay $100 per month, and adults 65 and older pay $125 per month. A family plan covering two adults and two children costs $200 per month. Additional children up to age 25 add $10 per month each.

The membership covers comprehensive primary care with no copays or per-visit fees. It includes annual physicals, well-child visits, unlimited office visits, same-day or next-day appointments, chronic disease management, and in-office procedures such as laceration repair, joint injections, cryotherapy, skin biopsies, nebulizer treatment, ingrown toenail removal, ear wax removal, and EKGs. Members also have direct access to Dr. Stephens by phone, text, email, secure messaging, and video visit.

No. Stephens Family Medicine does not bill insurance for primary care services. Many members choose to keep a high-deductible insurance plan for hospitalizations, specialist visits, imaging, and emergencies. Dr. Stephens helps coordinate care when insurance is involved outside the practice.

In many cases, yes. DPC memberships are often eligible for HSA and FSA funds. Stephens Family Medicine recommends confirming eligibility with your HSA or FSA administrator or a tax professional before applying those funds.

You can cancel at any time. Your membership ends at the close of the current billing month. There are no long-term contracts. If you cancel and decide to re-enroll later, a $400 re-enrollment fee applies.

Yes. Stephens Family Medicine offers same-day or next-day appointments for urgent concerns. The practice limits its panel size specifically to make this level of access possible.

Yes. Dr. Stephens offers video visits as part of the membership. Members can also reach him directly by phone, text, email, and secure messaging for questions that do not require an office visit.

Yes. Medicare patients are welcome at Stephens Family Medicine. The practice does not bill Medicare for services covered under the membership. Many Medicare patients keep Part B for labs, imaging, specialists, and hospital care. Dr. Stephens is happy to discuss how your specific coverage fits the model.

The membership does not cover immunizations, medications, lab work, radiology and imaging, pathology fees, specialist visits, physical therapy, surgical procedures, hospitalizations, or emergency room visits. Lab work and imaging are available at discounted cash rates. All out-of-membership pricing is transparent and discussed in advance.

Stephens Family Medicine opens June 1st, 2026. Patients who already know Dr. Stephens can schedule directly with the new practice. New patients can schedule a Meet and Greet appointment to learn more before enrolling.
