Zai Health offers three primary care membership tiers. Individual membership costs $150 per month. An individual plus a spouse or child up to age 26 runs $250 per month. A family of three or more pays $300 per month. A separate comprehensive addiction medicine membership is available at $300 per month.

Membership is not required at the Ojai location. Non-members can walk in and receive care starting at $99 per visit. The Thousand Oaks location operates on the membership model, though prospective patients should contact the practice directly to discuss their options.

The Thousand Oaks location is open Monday through Friday from 9am to 5pm, Saturday from 9am to 9pm, and Sunday from 10am to 6pm. The Ojai location operates Monday through Friday from 9am to 3pm and is closed on weekends.

Yes. Zai Health provides telemedicine visits for primary care and also offers Tele-MAT, a telemedicine-based medication-assisted treatment program for patients managing addiction. Virtual visits allow patients to connect with Dr. Rezai without coming into the clinic.

Members receive covered visits for primary care, urgent care, preventive care including annual exams, and chronic disease management. Basic on-site lab tests such as pregnancy, strep, flu, and urinalysis testing carry no additional charge. Members also receive discounted or included pricing on other services such as IV therapy, diagnostics, and in-house pharmacy access.

Yes. Zai Health runs a comprehensive addiction medicine program available both in person and through Tele-MAT. The addiction medicine membership costs $300 per month. Dr. Rezai holds board certification in Preventive Medicine with a specialization in Addiction Medicine.

Zai Health offers several IV therapy options including an Immune Infusion, NAD+ Infusion, Cold and Flu Infusion, and Myers Cocktail. Each infusion is customizable based on individual patient needs. Patients should plan for 45 to 90 minutes per treatment session.

Yes. Zai Health provides on-site x-ray and basic laboratory testing. Basic tests such as pregnancy, strep, flu, and urinalysis are included for members at no extra charge. A radiologist reads x-ray images and charges a separate fee for that service.

Yes. Dr. Rezai sees patients in nursing homes and assisted living facilities. This service extends care to patients who cannot easily travel to a clinic location.
