Individual memberships start at $90 per month for adults ages 18 to 64. A one-time enrollment fee of $50 applies. Families pay a maximum of $250 per month to cover an entire household. Membership requires a 6-month commitment, with a 90-day cancellation policy.

Individual members receive up to 36 visits per year, with a maximum of four visits per month. Membership covers in-office and telehealth visits, same or next-day urgent appointments, simple in-office procedures, direct messaging with Dr. Chiou, and health coaching. The practice charges no copays, no hidden fees, and no surprise bills.

The practice accepts patients regardless of their insurance status. Membership fees run outside of insurance billing. For labs and imaging, the practice partners with local companies to offer discounted cash-pay rates that often cost less than insurance-based pricing.

Yes. Presence Direct Primary Care offers same or next-day appointments for urgent concerns. Members can also reach Dr. Chiou directly by phone, text, or video at no extra cost, so urgent issues rarely require a trip to an outside urgent care clinic.

Yes. The practice offers virtual appointments alongside in-person visits. Members choose whichever format fits their schedule. Telehealth visits carry the same flat-fee structure as in-office care, with no additional charges.

Yes. The practice offers a dedicated weight loss support membership at $100 per month. That membership includes weight management support, nutrition coaching, and lifestyle change guidance. Dr. Chiou also supports GLP-1 therapy at a significant cost reduction compared to retail weight loss clinics, though medication costs fall outside the membership fee.

Yes. Presence Direct Primary Care offers an employer-focused DPC membership for companies that want to provide their employees with accessible primary care. The model emphasizes prevention and wellness, which helps employees stay healthy and reduces the need for costly outside medical interventions.

Yes. The practice offers occupational and regulatory exams to non-members as well as members. These include Police and Firefighter Physicals, Pre-Employment Exams, Veteran Military Medical Disability Exams, and Drug and Alcohol Testing. DOT/CDL Physicals and FAA Pilot Exams are coming soon.

Prospective patients can schedule a free meet-and-greet with Dr. Chiou before committing to a membership. This visit gives both the patient and Dr. Chiou a chance to connect and discuss health goals. To enroll, patients complete the online signup and pay the one-time $50 enrollment fee along with their first monthly membership payment.
