Planted Direct Primary Care charges one flat monthly membership fee with no hidden costs. That fee covers all included services, from annual wellness visits and chronic condition management to in-office testing and prescription management. Members also receive discounted rates on osteopathic treatments.

Appointments run 45 minutes. Dr. Koehn schedules unrushed visits so there is adequate time to address your concerns thoroughly. Same-day or next-business-day appointments are available for acute needs.

Yes. Members have direct access to Dr. Koehn by secure text, phone, and portal messaging. The all-in-one patient portal also gives you access to your chart, test results, billing, and messaging in one place.

Yes. Dr. Koehn offers both phone and video telehealth appointments. Virtual visits work the same way as in-person appointments for many types of concerns.

Planted Direct Primary Care operates outside of insurance. The practice charges a flat monthly membership fee, which covers all included services. Patients may still carry insurance for hospitalizations, specialist visits, or prescriptions not covered by the membership.

The membership covers annual wellness visits, preventive health counseling, chronic condition management, acute sick visits, prescription management, lifestyle medicine and nutrition coaching, basic mental health care, stress management, work and school or sports physicals for patients 16 and older, hospital discharge follow-up, coordination with subspecialty care, and in-office testing for urine, strep, flu, and COVID.

The practice serves adults. Dr. Koehn built it specifically for people managing demanding schedules, careers, and caregiving responsibilities for children, aging parents, or both.

You can enroll directly through the practice's online portal or schedule a free Meet and Greet with Dr. Koehn first. The Meet and Greet gives you a chance to learn more about the practice and ask questions before committing to membership.

Yes. New patients can use code DAD for $99 off enrollment before June 30th. The practice also offers waived enrollment for moms through May 31st with code MOM.
