Momentum Medicine

Individual memberships start at $65 per month for adults ages 19 to 39, $80 per month for ages 40 to 64, and $95 per month for ages 65 and older. Children ages 0 to 18 pay $50 per month. A family of four starts at $225 per month. College students can join for $300 per semester or $500 per year. A 5% discount applies when you pay annually. A $150 re-enrollment fee applies if you leave and return, with one exception for established snowbird patients.

The membership covers all visits and basic in-office procedures, including annual physicals, sick visits, injury care, sports medicine consults, joint injections, and ultrasound-guided joint injections. It also includes a basic annual lab panel with CBC, comprehensive chemistry, and lipid panel at no added cost. Members receive direct physician access by phone, text, and email, plus same-day and next-day appointment availability.

Yes. Starting January 2026, Momentum Medicine accepts HSA payments for monthly memberships. FSA funds also qualify in some instances. Check with your FSA plan administrator to confirm eligibility before applying those funds.

Momentum Medicine does not bill insurance for covered primary care services. The practice encourages members to maintain a separate catastrophic or high-deductible insurance plan for hospitalizations, surgeries, and specialist care outside the scope of primary care.

Members contact their physician directly by phone, text, or email. The practice provides after-hours access for truly urgent concerns. Non-urgent messages receive a response the next business day.

Yes. Momentum Medicine offers telemedicine visits for situations where virtual care fits. Dr. Erickson-Adams also offers home visits for members living in and north of West Olive.

When you enroll, you choose one physician as your primary doctor. Momentum Medicine prioritizes continuity of care, so you see your chosen physician at your visits. Family members may choose different doctors within the practice.

The membership includes one basic annual blood panel covering CBC, comprehensive chemistry, and a lipid panel. Additional labs carry transparent, low out-of-pocket prices. Examples include CBC at $3 to $4, HbA1c at $5, TSH at $6 to $7, and iron studies at $7 to $8.

Yes. The practice serves Holland, Zeeland, Grand Haven, and surrounding West Michigan communities from its office at 259 Hoover Blvd, Suite 100, Holland, MI 49423. Dr. Erickson-Adams also offers home visits for members in and north of West Olive.

Both physicians begin accepting patients in May 2026. Dr. Erickson-Adams currently accepts bookings, and Dr. Hilton opens his schedule in May 2026. Prospective patients can book a free introductory consultation through the practice website to learn more before enrolling.
