The Direct Primary Care option connects patients with Angela Burch, PA-C, with monthly fees ranging from $35 to $90 based on age, plus modest per-visit fees. Both tracks include same-day access and telehealth.

DPC monthly fees vary by age. Patients 25 and under pay $35 per month. Ages 26 to 44 pay $55. Ages 45 to 64 pay $75. Patients 65 and older pay $90. College students can enroll for $350 per year. Households with three or more members receive a 10% discount on membership fees.

Concierge members pay an annual fee of $3,450 and receive all office visits with Dr. Birkholz at no additional charge. Membership includes 24/7 direct access to Dr. Birkholz by cell phone, text, and email. Concierge members also receive annual body composition and metabolic rate analysis. Home visits are available for $300.

Laurie Birkholz MD & Associates operates outside of traditional insurance networks. Membership fees do not run through insurance. Patients with insurance can still use it for outside services such as specialist referrals, imaging, and hospital care. The practice offers deeply discounted in-office labs and medications as an added benefit.

EMPOWER is a 6-month personalized weight management program led by Angela Burch, PA-C. It combines nutrition guidance, fitness recommendations, FDA-approved obesity medication management, and accountability coaching. The program costs $400 for Concierge members, $550 for DPC members, and $950 for non-members.

Yes. Laurie Birkholz MD & Associates offers the Galleri Multi-Cancer Early Detection Test. This blood draw screens for signals shared by more than 50 cancer types, including many without routine screening options. The test is self-pay and eligible for HSA and FSA funds.
