Maple Primary Care

Membership fees at Maple Primary Care depend on the patient's age. Ages 5 to 19 pay $75 per month. Ages 20 to 39 pay $100 per month. Ages 40 to 59 pay $125 per month. Patients 60 and older pay $150 per month. Members who pay annually receive a 10 percent discount. A one-time $100 registration fee applies at enrollment.

The membership covers unlimited office visits, including an annual physical. It also includes unlimited telemedicine via phone, video, email, and text. Basic in-office labs such as rapid strep tests, EKGs, vision and hearing screenings, and urine dipsticks are all included. Common in-office procedures, including laceration repair, joint injections, cryotherapy, skin lesion biopsies, abscess drainage, and orthopedic splinting, carry no additional charge. Members also receive after-hours access to Dr. Kainth for urgent medical questions.

Members access labs through Maple Primary Care at rates averaging 80 to 90 percent below commercial prices. A CBC costs $10, a CMP costs $15, an HbA1c costs $15, and a lipid panel costs $15. Labs run through Quest or Clinical Pathology Laboratories. Members can also choose to submit certain labs, such as PAP smears and STI testing, to their insurance instead.

Yes. Dr. Kainth provides after-hours access for urgent medical questions. Members communicate with her directly, not through a portal or phone tree. She personally addresses urgent concerns outside of business hours.

Yes. The practice offers unlimited telemedicine as part of the membership. Dr. Kainth conducts visits by phone, video, email, and text. The enrollment process is available in both English and Spanish.

The practice does not bill insurance for membership services. Members pay the monthly fee directly. For certain services, such as PAP smears, mammograms, and STI testing, members can choose to submit lab or study costs to their own insurance.

Yes. Non-members can access a one-month service package for $300. Surgical clearance runs $200 and includes a detailed history and physical, EKG, CBC, and CMP. Comprehensive STI testing costs $350 and includes a mobile lab collection team that comes to the patient. Polygenic risk score genetic testing costs $500 and includes a detailed report and a virtual visit with Dr. Kainth.

Yes. Groups of five or more employees receive a waived registration fee and a five percent membership discount. Groups of ten or more employees receive a waived registration fee and a ten percent discount. Employers interested in offering Maple Primary Care to their teams can contact the practice directly.

New patients fill out an enrollment form on the Maple Primary Care website. The form is available in English and Spanish. After submitting, someone from Dr. Kainth's office will respond within two business days. No payment is due until the initial consultation with Dr. Kainth by phone, video, or in-person visit.

The practice sees patients ages five and older. Dr. Kainth provides pediatric primary care, including well child exams, sports physicals, and school and camp physicals. She also sees adult patients for primary care, women's health, chronic disease management, and preventive care.
