BloomMed membership starts at $140 per month, with quarterly and annual payment options also available. The annual plan totals $1,596. BloomMed also offers family and military discounts. Members who prefer flexibility can use a la carte pricing with no contract required.

Membership covers wellness exams, acute and sick visits, follow-up appointments, telemedicine visits, minor procedures, joint injections, and access to discounted labs. Members also receive an annual sports physical, the Flourish Health and Wellness Program, an initial InBody 570 body composition scan, in-house phlebotomy, EKG, spirometry, strep and flu testing, routine flu vaccines, antibiotic and pain medication injections, remote patient monitoring of chronic conditions, urgent after-hours phone access, and 10% off OrthoMolecular Products supplements. BloomMed also invites members to exclusive practice events.

Yes. BloomMed conducts telemedicine visits through the Spruce App, a HIPAA-compliant platform with two-way HD video. Members can schedule telehealth appointments alongside in-person visits. The Elation Passport patient portal also allows members to send secure non-urgent messages to Dr. Cudjoe.

Yes. Every Tuesday and Thursday, Dr. Cudjoe and medical assistant Lynette Jackson travel to patients' homes with a full clinical kit. House calls give members the option to receive care without leaving home. Contact the practice directly to arrange a home visit.

BloomMed operates on a direct primary care model and does not use insurance for membership fees. The practice does accept in-network assignment for certain insurance plans, including Blue Cross Blue Shield, Cigna, United Healthcare, Ambetter, and others. BloomMed does not contract with Medicare or Medicaid. For supplemental coverage, the practice partners with HSA for America, Summit Cost Share, Spirit Dental, Direct Vision, and Clever RX.

BloomMed welcomes patients from age 1 to 110. Dr. Cudjoe provides comprehensive family medicine care across all life stages. Visiting relatives of existing members can receive care as guests at a la carte rates.

Dr. Cudjoe manages a broad range of chronic conditions, including Type 2 diabetes, hypertension, hypothyroidism, asthma, and depression. She takes a root-cause approach to every health concern. Remote patient monitoring allows ongoing tracking of chronic conditions between visits.

Yes. BloomMed offers the Flourish Weight Loss Program for eligible patients. The program features compounded Semaglutide starting at $350 per month and Tirzepatide starting at $600 per month. It includes an initial consultation, monthly monitoring appointments, and lab work. Members also receive an initial InBody 570 body composition scan as part of their membership.

Yes. BloomMed offers several medical aesthetics treatments, including AquaFirmeXS medical grade facials, DE|RIVE Scalp Restoration Treatment, EvolveX Transform body contouring, and Morpheus 8. BloomMed accepts CareCredit for health, wellness, and beauty services.

BloomMed accepts new patients on a rolling basis. New patients should call or text the practice at (972) 214-8579 to schedule a first appointment. The online booking portal on the website serves only established patients. BloomMed also offers a la carte visits with no membership contract for those who prefer a pay-per-visit option.
