About
At the Freeman Center for Maximal Health, Dr. Earl R. Freeman, Jr. brings together Functional Medicine, Direct Primary Care, osteopathic care, and Addiction Medicine under one roof. The practice operates out of Dr. Freeman's home in Alfred, Maine. His approach centers on finding the root causes of disease rather than managing symptoms alone.
Dr. Freeman draws on decades of solo rural practice and applies the Institute for Functional Medicine's structured framework to each evaluation. He uses a three-phase process: a deep historical review, personalized testing, and individualized recommendations. The practice runs on a self-pay model with no insurance involvement. DPC members receive 24/7 physician access, same-day appointments, and discounts on Functional Medicine services. The DPC panel is currently full, though the practice maintains a waiting list for new primary care patients.

A Functional Medicine consultation costs $1,800. That fee covers more than ten hours of professional time, including the initial evaluation and three- and six-month follow-up visits. Dr. Freeman begins with a thorough intake process, including detailed medical history questionnaires and, when possible, a review of past medical records. The first in-person visit runs about two hours and includes a physical examination. Dr. Freeman then orders personalized lab testing based on what the history reveals. After results return, he delivers a Functional Medicine prescription with individualized recommendations. Follow-ups occur at three and six months.
Dr. Freeman communicates lab results within 48 to 72 hours of receiving them. He contacts patients by phone, email, or secure text. The practice draws lab specimens in the office, so patients do not need to visit a separate lab facility.
The practice operates out of Dr. Freeman's home in Alfred, Maine. Patients can reach the office by phone, text, or email. O. Box 849, Alfred, ME 04002.
