Five Journeys offers three membership tiers. The Standard plan starts at $129 per month and includes up to one-hour initial visits, 30-minute follow-ups, a 25% discount on wellness services, and a free musculoskeletal health assessment. The Silver plan runs $399 per month and adds priority booking and priority wellness scheduling. The Gold plan at $699 per month includes up to 90-minute initial visits, same-day telehealth, concierge coordinator services, and two complimentary wellness services plus three performance and recovery coaching sessions per year. The practice bills all appointments to insurance separately, and deductibles may apply.

The team performs some Lyme and food sensitivity tests onsite and offers Galleri cancer screening. Nutrient, vitamin, hormonal, microbiome, cortisol, and food sensitivity testing are all available.
