Epiphany Health Direct Primary Care

Adult membership costs $93 per month for patients under 65, and $100 per month for patients 65 and older. Dependent children from age 5 through their 26th birthday pay $30 per month for the first child and $15 per month for each additional child, when a paid adult membership is active. Epiphany Health also offers a 5% discount for annual payment made in full by cash or check.

The membership covers 25 office visits per year, an annual wellness exam, telemedicine visits, same-day and next-day appointments, extended appointments, and in-office procedures and testing without copays. Members also receive annual women's wellness exams and one annual fecal occult blood colon cancer screening. Additional office visits beyond the first 25 cost $25 each.
