Membership costs $125 per month per child, from birth through age 26. A one-time non-refundable enrollment fee of $100 applies. Families with multiple children pay $100 per additional child, with a maximum of $300 per family per month. Families who pay annually receive a 10% discount and a waived enrollment fee.

Creative Care Pediatric Health & Wellness does not accept or bill insurance for its services. The practice uses a transparent monthly membership model with no surprise bills. Families can still use their insurance for medications, labs, imaging, and emergency or specialty care outside the practice.

Starting January 1, 2026, federal law allows HSA funds to pay for direct primary care memberships as qualified medical expenses. Individuals can use up to $150 per month and families up to $300 per month in HSA funds toward DPC fees. This change makes Creative Care's membership more accessible for families with HSA accounts.

Yes. Dr. DeForest offers telehealth visits for members who need remote access to care. She also offers a special 4th Trimester Package, which includes home visits during the first four months of a newborn's life for an additional one-time fee of $500 per newborn.

Yes. Creative Care offers a single sick visit or evaluation for $245 without a membership. The practice also offers special standalone consultations: an ADHD evaluation for $300 and an Autism Spectrum Disorder evaluation for $650. Each includes a comprehensive diagnostic report and a follow-up visit with Dr. DeForest.

The practice offers same-day or next-day appointments for members. Members also have direct access to Dr. DeForest via clinic visits, telehealth, phone, and text messaging, which helps keep children out of urgent care or the emergency room for issues that Dr. DeForest can manage directly.

Creative Care limits its panel to 250 families. That cap allows Dr. DeForest to spend meaningful time with each child and respond thoroughly to each family's needs.

At this time, Creative Care cannot enroll children covered under Medicaid. The practice expects to open Medicaid eligibility around August 2027. Families in this situation can still schedule a discovery call to learn more and get guidance toward other available resources in the interim.

Dr. DeForest helps coordinate referrals and communicates directly with your child's specialist team when outside care is needed. The practice recommends that all families maintain health insurance for specialty and hospital services, as those fall outside the membership. Having a physician who knows your child well supports smoother, more coordinated care throughout the process.
