Cornerstone Family Medicine with Direct Care

Adults between 18 and 64 pay $70 per month. Children 17 and under pay $35 per month, and students in college, trade school, or graduate programs pay $35 per month. Seniors 65 and older pay $50 per month. Couples can join together for $100 per month. A family plan covering both parents and children 22 and younger costs $120 per month. Members who pay annually receive a 10% discount.

The membership covers one physical exam, follow-up visits, and up to 12 sick visits per year with no copayments. It also includes a range of in-office procedures at no additional charge. Those procedures include EKGs, minor laceration repair, skin biopsies and excisions, abscess drainage, wart freezing, IUD insertion and removal, and colposcopy for abnormal pap smears.

Yes. The membership does not replace insurance. You will still need a health insurance plan for hospitalizations, surgeries, specialist visits, lab work, and imaging. The practice does not bill insurance for anything it handles in the office. Membership fees do not apply toward your deductible or HSA, though the practice may help you save on labs and medications through lower-cost options.
