Community Primary Care

Adult memberships start at $94 per month. Children under 18 with an enrolled parent pay $49 per month. Couples plans start at $169 per month. Single-parent family plans start at $169 per month and cover one adult and up to two children. Two-parent family plans start at $249 per month for two adults and up to two children. Each additional child costs $39 per month.

Community Primary Care charges no enrollment fee. All memberships include a 3-month initial enrollment period so Dr. Highley can get to know each patient and build a real care plan. After the initial period, members can cancel anytime with 30 days written notice.

Billing begins on the date of your first visit, not when you sign up. The practice uses auto-pay billed monthly. Community Primary Care accepts credit cards, debit cards, HSA and FSA cards, and ACH debit. Cash payments are not accepted for membership fees.

In most cases, yes. HSA and FSA cards are typically accepted for DPC membership payments. The practice recommends confirming eligibility with your plan administrator before enrolling.

The monthly membership covers unlimited office visits with no copays, 30 to 60 minute appointments, same-day or next-day sick visits, direct access to Dr. Highley by call, text, and secure message, telehealth visits for established patients, in-office procedures, women's health, pediatric care, mental and behavioral health, chronic disease management, and care coordination with specialists. Starting April 1, 2026, the membership also includes over 400 prescription medications at no additional cost through home delivery and pharmacies nationwide.

The practice does not bill insurance. The monthly membership fee covers all included primary care services with no copays, deductibles, or surprise bills. Community Primary Care recommends pairing the membership with a health insurance plan or healthshare for hospital, emergency, and specialist coverage.

Starting April 1, 2026, every membership includes a prescription benefit through ManifestRx. Members receive over 400 maintenance medications delivered to their door at no additional cost, with 90-day supplies and up to 12 shipments per year. The benefit also covers 70-plus urgent care medications at over 70,000 pharmacies nationwide, with a $250 per-person annual maximum. For medications not on the included list, members receive a prescription discount card for use at retail pharmacies.

Members contact Dr. Highley directly by call, text, or secure message through the patient portal. The practice does not route messages through a call center or answering service. For established patients, telehealth video and phone visits are also available for follow-ups, medication checks, and certain sick visits.

The practice does not see Medicare patients at this time. Patients with Medicare who want to join can contact Community Primary Care to add their name to the waitlist. The practice will notify waitlisted patients when that changes.

New patients enroll online through the practice website. After completing enrollment, members receive a welcome email with instructions to access the patient portal and schedule their first visit. All new patients must complete their first visit in person to establish care. Telehealth visits for follow-ups and routine needs become available after that initial in-person appointment.
