Individual membership at Colab Concierge Physicians starts at $2,400 per year. Members pay no co-pays or deductibles for primary care services, and the practice does not bill insurance for those visits. Members can choose to pay monthly, quarterly, or annually.

The membership fee covers unlimited visits and direct access to your doctor. Routine visits run a minimum of 30 minutes and annual comprehensive exams run at least one hour. Members also receive 24/7 physician access, same or next-day appointments, house calls when needed, in-office blood draws, health planning, and referral coordination.

Yes. The membership covers all primary care services within the practice, but it does not cover hospitalizations, surgeries, specialist visits, or advanced diagnostics like MRIs and CT scans. Colab Concierge Physicians coordinates with your insurance for those services.

Compatible spending accounts including FSA, MSA, HRA, and HSA accounts may help cover the membership fee. We recommend checking with your account administrator to confirm eligibility for your specific plan.

Members receive their doctor's cell phone number for after-hours calls and texts. No answering service intercepts the call. You can also send secure portal messages through the Elation Passport patient portal for non-urgent needs.

Colab Concierge Physicians offers in-office blood draws, so members can handle routine lab work without going elsewhere. For advanced diagnostics not available in the office, the practice works with you and your insurance to find the most convenient and appropriate provider.

Yes. Sports medicine and musculoskeletal consultations with Dr. Niedfeldt do not require membership. Patients from across the country visit him for diagnosis and treatment of musculoskeletal injuries. Membership applies only to ongoing direct primary care services.

The practice sees patients ages 5 and older. Both Dr. Niedfeldt and Dr. Furey handle a broad range of primary care needs across age groups.
