All patients: $95 per month. Family discount: After the first 2 members of a family join, additional family members are only $40 each per month. Family Maximum $275 per month. A family includes up to 2 parents and all children age 20 and under. Children age 21 and older are not included in the family discount or the family maximum. If you cancel your membership and then wish to rejoin at a later time, you will be charged a re-enrollment fee of 4x the monthly fee at the time of re-enrollment.
