About

Dr. Jeff Horacek graduated with honors from the University of Oklahoma Medical School and went on to complete his Internal Medicine residency at Providence Portland Medical Center. He served as Chief Resident there from 1995 to 1996 and earned his board certification in Internal Medicine that same year. Medicine runs in his family: his grandfather practiced family medicine in rural Nebraska and his mother worked as an RN in rural Oklahoma.

After more than two decades in internal medicine, Dr. Horacek attended the Applying Functional Medicine to Clinical Practice conference in 2015. That experience redirected his focus toward whole-person care for patients with complicated, chronic illness. He holds the Institute for Functional Medicine Certified Practitioner designation and brings over 30 years of clinical experience to his work. He co-founded the non-profit Mindful Medicine PDX, which provides mindfulness training and workshops for healthcare providers, and served on the board of Living Yoga. He founded Healthy Connections in Hood River in 2018.
