Healthy Connections runs a hybrid model. The practice bills your in-network health insurance for office visits, so co-pays apply as usual. Your membership covers services that insurance does not, including health coaching, nutrition counseling, lifestyle programs, and Functional Medicine education. The practice offers two tiers: Short Term Functional Medicine Memberships and Primary Care Memberships.

Initial appointments run 75 minutes. This gives your doctor time to review your full health history and develop a thorough, individualized plan. Before booking, prospective patients can schedule a free 15-minute discovery call with the New Patient Coordinator.

Yes. The practice offers secure video and phone visits for established patients. Your first visit ideally takes place in person, and the practice asks that patients come in at least once per year in person.

Healthy Connections sees adults and adolescents aged 17 and older. The practice does not see children under 17. Parents or guardians must be involved in the care of adolescent patients.

The clinical team treats a broad range of chronic and complex conditions, including autoimmune disease, diabetes, metabolic syndrome, SIBO, hormone imbalances, adrenal dysfunction, chronic fatigue, gluten sensitivity, digestive problems, depression, and anxiety. The practice takes a functional medicine approach, focusing on root causes.

Yes. The practice partners with 3x4 Genetics to offer personalized genomic analysis. This testing helps the clinical team build more precise, individualized care plans based on your genetic profile.

Active members access several ongoing programs. Mindful Mondays offers weekly virtual mindfulness sessions on Zoom every Monday at 1 pm PT. Nutrition specialist Kirsten Chesney hosts virtual office hours every Thursday from 8:00 to 9:00 am. Members also access the Vitality Reset 30-day challenge at no additional cost. The Evergreen Vitality Reset program gives members an ongoing option year-round.
