Westfield Premier Physicians offers Standard Memberships starting at $60 per month for patients up to age 22. Adults ages 23 to 39 pay $110 per month, ages 40 to 49 pay $160 per month, and members 50 and older pay $210 per month. Families with multiple children between 23 months and 22 years pay $50 per month for each additional child after the first. The practice also offers a Platinum Membership at $260 per month for all ages, which adds an executive physical, advanced cardiac testing, injections, and standard labs to all standard benefits. Members can pay monthly, quarterly, semi-annually, or annually.

Every Standard Membership includes unlimited physician visits, telehealth access, in-office procedures, discounted lab tests, discounted prescription medications, and discounted imaging referrals. Members also receive FAA Aviation Medical Exams and sports physicals at no additional charge. The Spruce app gives members secure messaging and video visits after hours. The Platinum Membership adds an executive physical, extensive advanced cardiac testing, injections, and standard labs.

Westfield Premier Physicians does not bill insurance for membership fees. The practice operates outside insurance networks and uses a transparent, cash-based pricing model. Members who carry insurance can still use it for specialist visits, outside testing, and hospitalizations. The practice also welcomes Medicare patients, who pay the membership fee directly and keep Medicare active for services outside the practice. The practice accepts HSA, FSA, and HRA payments.

Yes. The practice uses the Spruce app for after-hours communication. Members can securely text, video chat, and share photos with their physician through the app. This gives members direct access to their doctor for urgent concerns without requiring an office visit.

Appointments at Westfield Premier Physicians last up to 60 minutes. That time allows the physician to fully explain health concerns, review treatments, and address any questions. The practice also offers same-day and next-day visits for urgent needs.

Yes. The practice covers the full age spectrum, from newborns through older adults. Dr. Robert Habig and Dr. Mary Pat Forkin both practice family medicine. Dr. Elaine Habig focuses on internal medicine for adults. Dr. Lindsay Moore specializes in both internal medicine and pediatrics, and she also provides breastfeeding and lactation care.

Each physician at Westfield Premier Physicians caps their panel at 600 patients. That limit allows physicians to offer same-day appointments, longer visits, and consistent availability for their members.

Yes. Dr. Robert Habig holds certification as an FAA Aviation Medical Examiner and performs flight physicals for pilots. The practice offers Aviation Exams at no charge to members. Non-members pay $165 for a standard Aviation Exam or $295 for an Aviation Exam with EKG. DOT physicals are also available to both members and non-members.

Achieve is the practice's medical weight management and aesthetic services program. It uses bioimpedance analysis and resting metabolic rate testing to personalize weight loss plans. Aesthetic services include Botox, dermal fillers, sclerotherapy, chemical peels, cryotherapy, and medical-grade skincare through ZO Skin Health. Dr. Elaine Habig serves as Achieve Medical Director. Hormone replacement therapy for women and men is coming soon.

Prospective patients can sign up for a membership directly through the practice's online enrollment. After joining, new members download the Spruce app for secure messaging and schedule their first appointment by phone, email, or text. The practice sends a medical history form to complete before the first visit. After that initial visit, members receive access to the Elation patient portal for their health records.
