Each member receives a one-hour comprehensive physical examination annually. The practice bills insurance for appropriate evaluation and management codes. Members also receive an annual Wellfit Fitness Evaluation at the Monterey Sports Center as part of the retainer benefits.

Yes, through the optional Executive Plan. This plan serves patients who have difficulty traveling to the office due to physical or medical limitations, and patients receiving hospice care. Contact the office to ask about adding the Executive Plan to your membership.

The practice limits its total panel to 500 patients across both physicians. Dr. Kubica manages her established panel, and Dr. Kelly Reed is actively building a new panel of her own. This panel limit allows each physician to provide timely, thorough appointments for every member.

Yes. The practice offers palliative care counseling, hospice coordination, and counseling on Advance Healthcare Directives and POLST orders. Dr. Kubica remains involved as the physician in charge through the hospice process and can arrange urgent home visits to help patients avoid unnecessary hospital or emergency room visits.
