Summit Primary Care

Monthly membership at Summit Primary Care costs $65 for patients ages 0 to 26 and $112 for patients ages 27 and older. Families, couples, and partners receive a 10% discount. The membership covers unlimited office visits, email access, telephone and video consultations, same-day appointments, and free or significantly reduced basic laboratory costs, with no co-pays.

Summit Primary Care does not accept health insurance of any kind. The practice recommends that members carry catastrophic or wrap-around coverage for care outside the office. The practice has also partnered with Sedera Health, a medical cost-sharing community that integrates well with the direct primary care model.

Yes. Summit Primary Care provides 24-hour physician access, seven days a week, 365 days a year. Members can reach Dr. Keller by phone, email, or through the Elation Passport patient portal for both urgent and non-urgent questions.

Yes. Dr. Keller offers house calls for an additional fee of $100. Members can request a house call for illness, injury, or other urgent needs that make an office visit difficult.

Yes. Members have access to unlimited video consultations through a secure, private channel. Telephone consultations are also included at no additional charge as part of the membership.

Membership covers annual comprehensive preventive health exams, unlimited office visits, basic office tests such as urinalysis, strep tests, and flu swabs, and simple dermatologic procedures including wart and mole removal, skin tag removal, and cryosurgery. Basic laboratory costs are free or significantly reduced for all members.

Yes. Summit Primary Care offers the Galleri Multi-Cancer Early Detection Test, which screens for more than 50 types of cancer through a simple blood draw. The test costs $1,000 for existing members and $1,200 for friends and family of existing members. Dr. Keller requires a pre-test consultation with every patient who considers the Galleri test.

Yes, for illness and injury visits. Summit Primary Care uses an open access model that allows members to walk in during office hours for urgent conditions. Routine and preventive visits require a scheduled appointment.

The practice is currently full. Summit Primary Care accepts new patients on a rolling basis from its waiting list. Prospective patients can submit their information through the practice, and the practice accepts them in the order they registered.

Yes. Members receive discounts through a network of partnered providers. These include 20% off chiropractic services, 10% off physical therapy visits plus free 15-minute consultations, 20% off acupuncture, 10% off the first three psychotherapy sessions, and up to 20% off fitness coaching. Members also receive discounts on Orangetheory Fitness monthly memberships.
