South Shore DPC charges $50 per month for members aged 18 to 50, $60 per month for ages 51 to 65, and $70 per month for ages 66 and older. All new and returning patients pay a one-time $100 consultation and enrollment fee. Members who pay a full year upfront receive one month free.

Membership covers unlimited acute and routine office visits, annual physicals with comprehensive labs, chronic condition management, sports physicals, weight management, mental wellness consultation, smoking cessation counseling and treatment, same-day appointments, and telemedicine. Members also receive access to wholesale lab pricing and reduced-rate radiology referrals.

Yes. South Shore DPC members can reach Dr. Elami after hours by phone, text, or email for urgent matters. Direct physician access is a core benefit of membership.

South Shore DPC operates on a direct membership model and does not bill insurance for membership services. Members pay a flat monthly fee with no copays or deductibles. Since DPC does not cover hospitalizations or specialist care, South Shore DPC recommends carrying separate coverage for those needs.

Yes. Dr. Elami conducts telemedicine visits for members. Members can also reach him by phone, text, or email, and he can address many concerns remotely, including locating pharmacies and sending prescriptions when a patient is away from home.

Yes. South Shore DPC memberships run month to month with no long-term contract. The practice asks for 30 days notice when possible, but members may cancel at any time. A cancellation form is available on the practice website.
