Suboxone (buprenorphine) opiate replacement therapy patients pay $150/month or $450/quarter regardless of age. OMT visits are fee-for-service: initial visits $108-165 (usually $137), follow-up visits $77-134 (usually $106). Retainer is not covered by insurance. Retainer paid for unused months is fully refundable.
