Revivify Restorative Medicine

Revivify DPC charges $129 per month for adults and $75 per month for dependents 18 and under. A family of four living in the same household pays a discounted rate of $375 per month. The practice does not accept insurance for DPC membership fees.

Members receive virtually unlimited office, phone, text, and video visits. The membership also covers same-day or next-day appointments with minimum 30-minute visits, annual wellness exams, EKGs, wound repair, splinting and casting, minor skin lesion removal, breathing treatments, abscess incision and drainage, and ear cleaning. Members also receive deep discounts on labs, imaging, generic medications dispensed in-office, and specialist referrals.

Revivify offers a free 10-minute Meet and Greet, available in person or virtually, for prospective patients to learn about the practice and determine if it is a good fit. Call the practice or reach out through the website to schedule yours.

The practice does not accept insurance for DPC membership services. Members pay a flat monthly fee directly to the practice. Dr. Daquino has opted out of Medicare, which allows him to bill through the membership model for Medicare-eligible patients.

Dr. Daquino hosts free Zoom webinars for business owners with one to 100 employees who want to explore DPC as an employee health benefit.
