Individual membership costs $4,500 per year, billed annually. Couple membership runs $7,500 per year. Families adding children pay $1,500 per child per year on an individual membership, or $1,000 per child per year on a couple membership. The annual membership covers house calls, telemedicine, biannual basic labs, point-of-care testing, basic procedures including supplies, and osteopathic manipulative treatment and cupping as indicated. The practice also performs remaining lab work and imaging at no markup. The membership fee is not insurance and does not cover emergency services.

Premier Village Physicians serves families residing in Fishers, Carmel, Westfield, Noblesville, Geist, and surrounding communities. The practice geographic boundary extends to within a 5-mile bordering radius of those cities. Families living outside that area may incur additional fees.
