Fishers Direct Family Care

Adults pay $80 per month. Children ages 3 to 20 pay $40 per month, provided at least one adult family member also enrolls. Every new patient pays a one-time enrollment fee of $100, capped at $200 per family. The monthly fee covers all office visits, online communications, and in-office testing with no per-visit charges.

The practice charges a nominal additional fee for in-office medications and procedures. Examples include a steroid injection, skin biopsy, wart removal, or laceration repair. Dr. Patel prices these at wholesale cost only, covering just the supplies or medication used. These charges run well below what a typical office or urgent care would charge.

The practice does not bill insurance for any membership services. Members pay the monthly fee out of pocket, though the practice accepts HSA payments. Dr. Patel does collect insurance information to help coordinate referrals and outside testing within your network. The practice bills third-party insurance for vaccines through its vaccine partner, VaxCare.

Yes. The practice offers same-day appointments. Office hours run Monday through Thursday, 8am to 5pm, and Friday, 8am to 4pm. After hours, members can reach Dr. Patel by calling the main office number for urgent needs or by messaging through the Hale app for non-urgent communication.

Yes. The practice offers e-visits and video visits through Skype, FaceTime, and the Spruce app. Members can use these for acute issues, follow-up care, and preventive counseling. Telehealth is included in the monthly membership fee at no additional charge.

The practice gives members two options. Dr. Patel can submit a lab order to a local lab and bill your insurance directly. Or, members can pay the practice directly using negotiated cash pricing, with most common labs priced under $10. The cash pricing option requires no insurance involvement and settles at the time of the visit.

Yes. Fishers Direct Family Care partners with a local imaging group that offers transparent flat-fee pricing. Members can access x-rays, ultrasounds, CTs, and MRIs at less than half of typical hospital pricing. Dr. Patel provides referrals to keep members within this discounted network.

Yes. The practice offers MAT for Opioid Use Disorder using buprenorphine/naloxone medications such as Suboxone, Bunavail, and Zubsolv. Dr. Patel combines medication with counseling and behavioral therapy. The initial fee is $250, which covers screening, record review, onboarding, and the first month of treatment. Ongoing monthly care runs $150.
