Port City Direct Primary Care

Port City Direct Primary Care charges a monthly membership fee billed at the end of each month. Older adults (40+) pay $120 per month, adults ages 18-39 pay $100 per month, and children ages 0-17 pay $30 per month when added to an adult membership. A one-time enrollment fee applies: $100 for individuals and $200 for families. The practice also offers a $20 monthly discount for each additional adult after the primary patient.

Membership covers a wide range of services at no additional charge. These include well and sick visits, an annual wellness and physical exam, in-office point-of-care testing, EKG, laceration repairs, lesion removals, nutrition and wellness coaching, and up to 20 in-person visits per year. Members also receive unlimited access to the practice via phone, text, and email, plus same or next day appointment availability.

Port City Direct Primary Care does not bill insurance for its services. The practice operates on a direct monthly membership fee. Members can maintain a separate insurance plan for hospitalizations or specialist care, but the practice handles primary care services directly.

Port City Direct Primary Care contracts with Accu Reference Medical Lab to provide comprehensive lab testing at significantly reduced prices. All in-office point-of-care tests, including rapid strep, Covid/Flu testing, urinalysis, and glucose checks, are included in the membership at no additional charge. Yes. The practice guarantees same or next day appointment availability for all members. This applies to both sick visits and other primary care needs. Members can also reach the practice via phone, text, or email for quick questions or follow-ups.

Yes. Port City Direct Primary Care offers telehealth visits in addition to in-person appointments. Members can connect with their physician by phone, video, text, or email, depending on the nature of their concern.

Yes. Children ages 0-17 join for $30 per month when added to an adult membership. If a child enrolls without an adult membership, the first child's monthly rate is $50. Both Dr. Cullura and Dr. Collins see patients across all age groups.

Port City Direct Primary Care cannot see Medicare-eligible patients at this time. This limitation stems from Medicare regulations that govern direct primary care arrangements. Patients who do not hold Medicare eligibility may enroll without restriction.
