Membership fees at Perspective Health vary by age. Children from birth to 18 pay $60 per month or $720 per year. Adults ages 18 to 55 pay $120 per month or $1,400 per year. Adults over 56 pay $150 per month or $1,800 per year. The practice does not accept insurance for membership fees.

Every Perspective Health membership includes same-day and next-day appointments, annual wellness visits, chronic disease management, sick visits, and personalized treatment plans. New patients receive 90-minute appointments. Established patients receive 60-minute visits. Members also get direct access to their physician and the nursing staff by phone, text, or email.

Perspective Health does not accept insurance for membership fees. Patients pay the practice directly. However, the practice can still bill insurance for labs and imaging services if you prefer that option.

Yes. Perspective Health offers discounted labs and imaging to members. If you prefer, the practice can bill your insurance for those services instead. Your physician also coordinates specialist referrals when needed.
