Yes. Dr. Lang accepts most commercial insurance plans, Medicare, and most Medicare Advantage programs. The practice bills your insurance for medical services and accepts the reimbursement. Your membership fee then covers any remaining copays, co-insurance, or deductible amounts tied to in-practice services.

Health Savings Accounts may cover the annual membership fee. The practice recommends consulting a tax professional or your workplace human resources department to confirm eligibility and any tax treatment.

Personalized Primary Care guarantees same-day visits for illness. For non-urgent matters, the practice schedules appointments the next day. Appointments start on time and run a minimum of 30 minutes, with no significant wait times.

Dr. Lang gives every member his personal cell phone number at enrollment. He is available 24 hours a day, 7 days a week, every day of the year. For urgent after-hours issues, he will meet patients at the office or at home. A backup physician covers the practice whenever Dr. Lang is out of town.

Personalized Primary Care treats patients from 7 years old through end of life. Spouses, domestic partners, and children on the family insurance plan up to age 26 can join under a family membership.

The annual physical at Personalized Primary Care runs approximately two hours. Dr. Lang performs an EKG, spirometry, ankle-brachial index testing, hearing testing, vision testing, and thorough lab work. He also calculates a cardiovascular risk score using the Pooled Cohort Equation and an osteoporosis fracture risk score using the FRAX tool, along with screening assessments for sleep, depression, anxiety, memory, activity, and nutrition.

Dr. Lang conducts house calls for urgent matters outside office hours. He will meet patients at the office or at their home when the situation warrants. The practice also provides a patient portal for non-urgent communication and electronic access to Cleveland Clinic System medical records.

Yes. Members who wish to leave the practice at any time receive a prorated refund of their membership fee.
