The monthly membership rate is $85 for patients of all ages. A one-time registration fee of $100 applies when joining. Home visits cost $150 per visit. State sales tax is included in all fees shown.

Membership covers annual preventive visits, unlimited sick and follow-up visits, an annual flu shot, and a full range of in-office procedures including urinalysis, strep tests, ECGs, ear wax removal, minor skin procedures, and albuterol treatments. Members also receive access to discounted medications and labs, and attend Dr. Abousleman's healthy cooking classes at no additional cost.

No. The practice does not accept insurance payments for care. Dr. Abousleman recommends that members keep their insurance or a health share plan to cover emergencies, hospital stays, specialist visits, and catastrophic care. DPC membership covers only the care Dr. Abousleman provides in her office.

PPO and standard insurance plans generally allow this. HMO, MCO, and Medicare Advantage plans may require an in-network provider to place those orders. Check with your insurer before enrolling. Standard Medicare and Medicaid allow Dr. Abousleman to order labs and referrals; managed versions through private insurers may not.

Yes. The practice offers same-day and next-day appointments for sick visits. Members choose between in-person, telephone, or video visits depending on their needs.

No. The practice does not require a long-term contract. Members can end their membership at any time with 30 days of written notice and no penalty. If a former member rejoins later, the registration fee and current pricing apply at that time.

The practice is currently full and not enrolling new patients. Prospective patients can join the waitlist by contacting the practice directly. The current estimated wait is approximately 10 months.

Dr. Abousleman personally teaches healthy cooking classes at the practice. Classes run approximately 60 to 90 minutes and cover a wide range of topics, including salads, stir-fry, plant-based proteins, whole grain dishes, and Green Chile Chicken Enchiladas. Members attend free of charge. Non-members pay $20 per class on a sliding scale.

Yes. Dr. Abousleman offers telephone and video visits. Patients choose the format that works best for their situation, and the practice accommodates both for most types of visits.
