Lotus Health membership starts at $95 per month. The practice charges a one-time $100 enrollment fee for new members. An optional hormone therapy add-on is available for an additional $100 per month. Two membership tiers exist: the Foundation tier covers basic healthcare needs, medication management, and virtual appointments; the Essentials tier adds chronic care management and in-office appointments.

The monthly membership covers primary care services, urgent care, medication management, chronic care, and basic procedures. Members also get direct access to Dr. Chan by phone, text, email, and video. The practice offers same-day or next-day appointments and extended visits of 30 to 60 minutes.

Lotus Health does not bill insurance for membership services. The DPC model allows patients to pay Dr. Chan directly, keeping costs transparent and eliminating copays. Patients may choose to maintain a separate insurance plan for services outside the practice's scope, such as hospitalizations or specialist referrals.

Yes. DPC members can contact Dr. Chan directly by voice call, text, email, or video. This direct line of access is one of the core benefits of the Lotus Health membership model.

Yes. Lotus Health provides virtual appointments for members. The Foundation tier includes virtual visits, and the Essentials tier adds in-office appointments. Telehealth allows the practice to serve patients both locally in Utah and nationwide.

Lotus Health serves patients from infancy through senior years. Child pricing applies to patients ages 12 and under, and a child's membership requires that a parent or guardian hold an active Lotus Health membership on the same plan.

Yes. Dr. Chan performs medical cannabis evaluations and renewals in Utah. The standard evaluation fee is $125. DPC members receive cannabis appointments included as part of their membership.

Medical Assisted Treatment at Lotus Health supports patients managing withdrawal and substance use disorders through evidence-based medication management. Dr. Chan collaborates with therapists and treats every patient with respect and confidentiality throughout the process.

Yes. Lotus Health partners with FullScript, giving members access to a 10% discount on supplements ordered through the practice's FullScript account.

Yes. Lotus Health provides a broad range of services including acupuncture, cupping, gua sha, acupressure, IV infusion therapy, intramuscular vitamin injections, well-woman care, hormone therapy, palliative care, ADHD evaluation and treatment, and precision longevity medicine. Dr. Chan also holds outreach clinics at community locations for medical cannabis card evaluations.
