Monthly fees depend on age. Children ages 0-18 pay $35 per month with at least one adult membership on the account. Adults ages 19-44 pay $65 per month. Adults 45 and older pay $85 per month. Liberty charges no initial enrollment fee and no long-term contracts. A $250 re-enrollment fee applies if a member cancels and later returns.

Starting January 1, 2026, members can use HSA funds to pay for their Liberty Family Medicine membership. This change makes the monthly fee eligible for tax-advantaged spending for many households.

Liberty does not replace health insurance but works alongside it. The practice partners with Liberty Insurance Solutions, run by Healthcare Advisor Jason Gruender, to pair clinic memberships with health insurance and medical cost-sharing plans. Businesses that combine membership with a Liberty Insurance Solutions plan save an average of $2,000 per employee per year.

Membership covers all office visits with no copays, telemedicine appointments, direct physician access by phone, text, email, and video, and over 150 generic medications at no additional cost. Members also receive wholesale laboratory and imaging rates and access to discounted local health and wellness businesses through the practice's MeliUS United partnership.

Liberty provides after-hours phone access for urgent concerns. During clinic hours, nurses answer all incoming calls directly. No automated phone systems or patient portals stand between members and the clinical team.

Yes. Liberty offers same-day or next-day availability for acute concerns. Most standard appointments run 30 minutes to one hour, and the practice keeps panel sizes between 600 and 800 patients per provider to support that access.

Yes. The practice offers telemedicine visits and home visits. Home visits carry a discounted fee separate from the monthly membership.

The physicians handle a wide range of procedures including laceration repair, cyst removal, abscess drainage, ingrown toenail removal, IUD insertion and removal, joint and trigger point injections, skin lesion and mole removal, endometrial biopsy, simple fracture care, ear wax removal, IV fluids, wart treatment, nebulizer treatments, and mole mapping with the Skin IO system.

Yes. Liberty accepts Medicare patients. The clinic does not bill Medicare directly; instead, Medicare patients pay the standard age-appropriate monthly membership fee.
