Membership fees at Horizons Direct Primary Care depend on age. Children ages 0-17 pay $75 per month, adults ages 18-49 pay $110 per month, and adults 50 and older pay $125 per month. A one-time enrollment fee of $100 applies to new members. The practice offers a 10% discount for annual memberships, a 10% discount for family members in the same household, and a 10% discount for full-time students.

The monthly membership covers most primary care services and in-office testing with no additional fees. This includes annual physicals, acute care visits, chronic disease management, gynecologic exams, well child checks, sports physicals, rapid testing for UTIs, COVID, flu, pregnancy, and strep, and a range of in-office procedures. Lab work carries a separate fee, but Horizons Direct Primary Care provides those services at discounted cash rates disclosed upfront.
