In-Person DPC membership costs $200 per month or $2,400 per year for patients ages 22 and older. Virtual DPC starts at $150 per month for the same age group. Virtual members do not incur no-show fees. Dr. Shabazz holds active licenses in PA, NJ, NY, GA, MI, TX, and IL, so virtual members in those states can receive ongoing primary care remotely.

Fountain Medical Associates does not accept commercial or ACA marketplace insurance for DPC membership fees. The practice does accept select Medicare and Medicaid plans, including Cigna Medicare, Health Partners Medicare, Medicare Part B, Health Partners Medicaid, and Kidz Partners. Patients with commercial insurance plans enroll directly in DPC. Members may still use their insurance benefits for services the practice does not provide directly, such as outside lab work or imaging.

Yes. The practice reserves same-day and next-day slots on the schedule for DPC members, available for both in-person and telehealth visits. DPC appointments run 20 to 60 minutes depending on what the patient needs. Members can also reach Dr. Shabazz via secure text or the after-hours access line outside of regular hours.

Services include well child exams, developmental screenings, school and sports physicals, adolescent health and counseling, and adult chronic disease management. Membership pricing reflects age, with reduced rates available for patients 21 and under.

Dr. Shabazz manages a wide range of chronic conditions including diabetes, high blood pressure, high cholesterol, asthma, COPD, coronary artery disease, vascular disease, and obesity.
