Evolve Healthcare charges $85 per month per adult member age 19 and older. Children ages 0 through 18 pay $20 per month, though a parent or guardian must also hold an adult membership. Membership fees cover office visits and a defined set of included services. Cherry Financing offers payment plans from three to sixty months, with qualifying 0% APR options available.

Members receive unlimited office visits, cryotherapy for warts and actinic or seborrheic lesions, cautery and skin tag removal, sutures, an annual wellness lab panel, PAP and HPV testing, urinalysis, strep testing, and pregnancy testing. Patients receive weekly semaglutide injections and attend weekly in-clinic monitoring sessions throughout the active weight loss phase. Most IV therapy treatments require a consultation and preliminary lab work before the first session. The Hangover Therapy is the exception and is available on a same-day basis without prior lab work.
