Members receive unlimited visits, direct access to their chosen physician by phone, text, or secure portal, and annual routine labs at no added cost. Appointments run 30 to 90 minutes depending on need, and same-day availability is the norm. The practice also offers telemedicine and house calls. Dr. Maus leads a virtual-only plan at $90 per month for Minnesota patients. Evergreen maintains a Patient Assistance Fund, directing 100% of contributions toward membership fees, labs, and clinic services for those facing financial hardship.
